Function set_thread_exited is shared from delete_thread_1. This patch also moves field "refcount" to private and methods inc_refcount and dec_refcount. Additionally, we stop using "ptid_t" in "struct current_thread_cleanup" to reference threads, instead we use "thread_info" directly.
