[v2.1] Support structure offsets that are 512K or larger.

This version fixes the small nits found during the review.  It will be
pushed shortly.

GDB computes structure field byte offsets using 32 bit quantities.
And it first computes the offset as a bit offset and then converts to
bytes.  This means that if the offset if 512K bytes or larger, it
overflows.  This patch changes GDB to use LONGEST instead.
